Regulatory factors and cytokines in serum and follicular fluid in women with controlled ovarian hyperstimulation

Pregnancy is associated with predominance of Th2/Th3 cytokines. However, it is still debatable whether the shift of cytokine balance is triggered during initial (for example, during embryo implantation) or late stages of pregnancy. The aim of the study was to evaluate the biological activity of follicular fluid (FF) and serum with the measurement of cytokines in FF, in women with controlled ovarian hyperstimulation following in-vitro fertilization (IVF). 30 IVF patients and 10 healthy women with regular menstrual cycle were examined. Biological activity of the women's sera at the beginning of natural menstrual cycle was similar to that in healthy donors, whereas in ovulation phase sera exhibited significantly enhanced immunosuppressive and anti-inflammatory activities. Suppressive, marked antiinflammatory, and apoptosis-stimulating activity was also revealed in IVF patients with controlled ovarian hyperstimulation both on systemic (in blood serum), and on local (in follicular fluid) levels. Nevertheless, the pro-inflammatory/immunostimulatory (IL-1a, TNFa, IL-8, IL-2) as well the antiinflammatory/immunosuppressive (IL-4, IL-1Ra) cytokines were detected in FF from women undergoing IVF. In individual cases the remarkable scatter of cytokines content was noted. However, IL-2 values correlated positively with the number of mature oocytes and embryo quantity (rS = 0,48 u rS = 0,55; p < 0,05 respectively). High level of IL-2 in FF was also associated with complicated course of IVF due to ovarian hyperstimulation syndrome. On the other hand, we found that the concentration of human gonadotropin - the early marker for successful embryo implantation and maturity — significantly correlated with enhanced immunosuppressive and anti-inflammatory activities of regulatory factors in FF. Obtained data suggest that the pro-inflammatory/immunostimulatory cytokines participate in maturation of oocytes and may be important for human pre-embryo development, whereas the following shift of regulatory balance toward the anti-inflammatory/immunosuppressive cytokines contribute to successful embryo implantation and may be necessary for the establishment of pregnancy in women undergoing IVF.
